Your First Visit

From the first phone call or walk-in, we here at Children’s Orthotics and Prosthetics are on your team. We understand that your a parent and are concerned or worried about your child’s care. That’s OK, we’re parents too and we’ll always treat your kids as children first and patients second.

At Children’s consultations and evaluations are always free, whether your infant needs a cranial remolding helmet or your child needs a post-operative prosthetic, we’re here to help you in every step of the way.

If you haven’t scheduled an appointment, please call the office at 702-932-1300. Our friendly office staff can assist you in setting up a time to talk with the practitioner and answer any questions you may have.

What to bring with you on your first visit.

Written Prescription or Referral.

We accept walk-ins and evaluations, but before we can begin actual treatment, we need a written prescription. In most cases, your pediatrician or physician will fax one to us. In the event you received a written prescription or referral, make sure to bring it with you.

Insurance.

Please bring all related and active insurance cards with you. Your insurance cards have critical information to speed up the billing process, such as: policy number, group number, plan codes, effective dates, co-pays, deductibles, referral/authorization information, physician phone numbers, insurance company phone numbers and other important information. These cards help our staff to identify your coverage, since many insurance companies offer different types of plans.

Identification.

We need a current and valid photo ID to process your paperwork.
